Must be a legal resident of the United States
Must be eighteen (18) years of age or older to participate, although the winner’s designated beneficiary may be younger than 18.
RULES AND RESTRICTIONS:
A ticket represents a chance to accept the stated prize(s) subject to all terms and conditions set forth herein and in the "Winner Affidavit”. Winning tickets shall be selected by a random drawing. Winner need not be present to win. Winners will be contacted in-person or by phone, notified by Certified/Return Receipt Mail, and will have fourteen (14) days from receipt to accept the prize and sign and deliver the “Winner Affidavit” to Hospital Sisters Mission Outreach per written instruction. In the event the winner fails to accept the prize within the time set forth (14 days), a second drawing will take place and a new winner announced, subject to the same rules, conditions and the affidavit.
All prizes remain the sole property of Hospital Sisters Mission Outreach until unconditionally accepted "as is, where is” by the winner. Prizes shall not be awarded until the “Winner Affidavit” has been signed and delivered to Hospital Sisters Mission Outreach.
Prizes will not be disbursed until all checks, and credit card payments are cleared.
The amount of the prize will not be certified until the aforementioned payments are cleared, and a determination of administrative costs has been made and deducted from the total amount the fundraiser earns.
The amount of the prize will be awarded to the winner after tax-withholding amount has been determined, and sent to the IRS.
All ticket purchases are final and non-refundable.
Void where prohibited by law
Prize will not be awarded and shall be forfeited if winner's ticket was issued in exchange for a check returned for "insufficient funds” or for a credit card charge declined, invalid, stolen, or otherwise rejected.
Except where prohibited by law, names and/or likenesses and quotes of winners may be disclosed to and used by the news media and may otherwise be used by Hospital Sisters Mission Outreach for publicity purposes and in lists of prize winners to be published in area newspapers, and announced on area television and radio stations and the Hospital Sisters Mission Outreach website.
Hospital Sisters Mission Outreach reserves the right, in its sole discretion, to disqualify any person that it finds to be or suspects to be (a) tampering with, or attempting to tamper with, the entry process or the operation of the raffle, or (b) acting in violation of the Official Rules, or with intent to abuse, threaten or harass any other person. Any attempt by a contestant or any other individual to deliberately damage or undermine any website or the legitimate operation of the raffle is a violation of criminal and civil laws. Should such an attempt be made, Hospital Sisters Mission Outreach or its’ affiliated organization, Hospital Sisters Health System, and its agents reserve the right to prosecute such person and/or seek damages from such person to the fullest extent permitted by law.

Hospital Sisters Mission Outreach’s interpretation and application of the rules and regulations shall be final.
LIMITATIONS OF LIABILITY:
Purchaser agrees to release Hospital Sisters Mission Outreach and its’ affiliated organization, Hospital Sisters Health System, its directors, officers, employees, and agents from any and all liability for injuries, losses, or damages of any kind caused by participating in the raffle or winning any prize or resulting from acceptance, possession, use, or misuse of any prize and each winner agrees to indemnify and hold Hospital Sisters Mission Outreach and its’ affiliated organization, Hospital Sisters Health System harmless from any and all losses, damages, rights, claims, and actions of any kind rising in connection with or as a result of participating in the raffle or the winner's acceptance or use of any prize.
A raffle participant's sole and exclusive remedy for a breach by Hospital Sisters Mission Outreach shall be limited to the return of the purchase price paid for his or her raffle ticket(s).
The COVID-19 response raffle tickets must be paid for prior to the drawing with a date yet to be determined.
Unpaid raffle tickets WILL NOT be included in the drawing.
No part of the purchase price of tickets is tax-deductible and winner is responsible for any tax liability incurred from prize.
Tickets will be sold as long as they last or through the date of the drawing on November 7, 2020.
**A Sangamon County Raffle License has been obtained for this drawing and a copy of this license can be
